When contacting the insurance company it is important to provide as much detail as possible, including the name of the insured, the policyholder’s Social Security or ID number and the mailing address.

Go to http://www.findyourpolicy.com and check out their database. If the insured was a member you will easily find the company name that they had life insurance with.
